We believe of a persistent and unique "self" because of a confusion :

Memory is about the physical process, and we believe it is about our "subjective world".
Then we believe the "subjective world" is persistent, when it is not in any meaningful way.
To bring a computer science analogy: A "self" can be thought of as a set of configurations in a neural network. This set gets trained over time, but remains the same object.
